Pediatric Integrated Health Program

At Four Oaks, we understand that you want the best for your child. We do too! That is why Four Oaks is pleased to offer a unique service for Linn, Benton and Buchanan County children enrolled in Medicaid. The Integrated Health Program helps coordinate services that your child needs to live a healthier life.  

The Integrated Health Program connects you with a care coordinator, who will work with you to address your child’s mental and physical health needs. Together, you can focus on your child’s total health.  The care coordinator can help you:

  • Understand and manage your child’s physical and behavioral health services
  • Create a whole health treatment plan just for your child
  • Provide support to you, your child and your family members
  • Make sure your child has access to care when he or she needs it
This free program supports your decision to ensure your child receives the best possible care and that he or she is healthy and happy. Four Oaks’ Integrated Health Program team is excited about this opportunity for your child, and team members are available to answer any questions you may have. Frequently Asked Questions

I received a letter indicating that Four Oaks is my Integrated Health Home Provider.  What do I do?
Please call us 319-784-2250 and connect with a care coordinator who is ready to assist with your child’s needs.  

How much does the program cost?
The program is free for Medicaid members. It does not change your child’s Medicaid benefits.  It simply adds to them.

Is my child eligible to participate in the Integrated Health Program?
We would be happy to check! Please call us at 319-784-2250 so we may help. 

Will we have to switch doctors?
No.  We honor our families’ choice of providers, and will support their decisions on who will provide the best care for their child. 

What service does Integrated Health provide?
Integrated Health provides care coordination. Our team will help you and your family manage your child’s whole health, including his or her mental health, behavioral health, and physical health services. We are focused on helping you and your child achieve positive outcomes.

What is the need for Integrated Health?
Mental health plays a major role in children’s ability to maintain good physical health. Studies show that children with mental health challenges often experience worse health outcomes than their peers. The good news is many of the conditions can be prevented. Four Oaks’ Integrated Health Program will team with you to help your child lead a healthy life.
